Ledger & Oak
SERVICES / REIMBURSEMENT

Get paid for the care you already deliver.

Cost reports and supplemental programs decide how much of a rural hospital's care is actually reimbursed. We make sure what you filed matches what your ledger says and that no program window closes on you unnoticed.

THE PROBLEM

Reimbursement and cost reports

Most small hospitals file a Medicare cost report every year and never look at it again until a desk review letter arrives. The report is built from the general ledger, so every mapping decision in the ledger flows straight into cost-to-charge ratios, uncompensated care calculations and settlement.

We start where the report starts. We compare your trial balance to what was filed, find expenses that landed in the wrong cost centers and tell you in dollars whether an amendment is worth the effort. When it is not worth it, we say so in writing.

THE WORK

What an engagement can include.

Scope is set in writing before we start. Most clients begin with one piece and add others once the first one proves out.

  • Medicare cost report review against the general ledger and trial balance
  • Expense remapping and cost-to-charge ratio analysis
  • Amendment go or no-go recommendation based on estimated settlement impact
  • Medicare bad debt process and data review
  • Medicaid days and Medicare crossover coordination
  • Texas uncompensated care and DSH applications and filing calendars
  • Worksheet S-10 and S-12 support
  • CAM and Report III for Texas behavioral health centers
  • Rural Health Transformation Program and other funding applications

We review and coordinate. Where another firm prepares your cost report, it stays the preparer of record. We do not guarantee settlement or payment outcomes.

HOW IT RUNS

Four steps, in writing.

01

Data access

We request the trial balance, the filed report and the supporting detail through a secure transfer under a signed BAA.

02

Review

We tie the ledger to the report line by line and flag remapping opportunities and data gaps.

03

Recommendation

You receive written findings with estimated impact and a clear recommendation on whether to amend.

04

Calendar

We keep the program deadlines in front of your team so nothing lapses between filing seasons.

PROOF

A rural hospital in Mississippi engaged us to write its Rural Health Transformation Program application. The state funded it in full.
